Computed tomography-guided lung biopsy (CT-GLB) is the current standard of care for a variety of lung diseases. The aim of this study was to assess the safety and efficacy of CT-GLB in patients with non-small cell lung cancer (NSCLC). A systematic review of the MEDLINE, Embase, PubMed, and Cochrane Library was performed through May 2011. Two independent reviewers extracted data and assessed the quality of evidence (Oxman classification). Thirty-six studies (8 prospective and 28 retrospective) with a total of 2310 patients were identified. Overall, the CT-GLB and surgical biopsy (SB) groups were similar with respect to several baseline characteristics. Most common complications were pneumothorax and haemoptysis. The overall diagnostic yield was 66.5% for CT-GLB and 68.4% for SB (p=0.63). Diagnostic yield of SB was significantly lower than CT-GLB in squamous cell carcinoma, lymphoma, sarcoma, and primary bronchogenic carcinoma. However, the diagnostic yield of CT-GLB was significantly higher than that of SB in adenocarcinoma, small cell lung cancer, and metastatic lung cancer. This review of published studies of CT-GLB in the evaluation of patients with NSCLC provides evidence for the safety and efficacy of this method, and supports the use of this procedure for obtaining adequate tissue in NSCLC.
